    SUBACUTE UNIT

    YOUR BRIDGE TO RECOVERY

    Our 40-bed subacute unit provides inpatient care to patients who can be discharged from a hospital, but require comprehensive, complex care and rehabilitation services. Our caring nursing and rehabilitative staff provides a bridge between hospital and home. The unit is located within our hospital, which enables us to provide care for a variety of complex diagnoses. Additionally, the subacute unit has the benefit of onsite physician services.

    Subacute patients usually require between 45 to 720 minutes of therapy a week. Our skilled and caring team works to meet individual patient goals. Services provided at the Cleveland subacute unit include C-PAP/Bi-PAP, peripheral and central line/PICC management, wound care, IV therapy management, pain management, respiratory care, tube feeding and management of an array of clinically complex care including pneumonia, stroke, orthopedic injury/surgery, diabetes and chemotherapy. The Cleveland subacute unit is Medicare certified.

    4 Star Rating

    Kindred Hospital Cleveland Subacute Unit has been honored with a Four-Star rating in September 2011 from the Centers for Medicare and Medicaid Services (CMS). CMS created the Five-Star Quality Rating System to help consumers, their families, and caregivers compare subacute units more easily and to help identify areas about which you may want to ask questions. The quality rating system gives each nursing home a rating of between 1 and 5 stars. Subacute units with 4 or 5 stars are considered to have “above average” quality.
